Twin Cities Aging Network
The disconnect between health care delivery and home- and community-based services outside of clinical settings is an ongoing challenge in the lives of older adults and their families. To address this growing challenge, an integrated network of over 25 home and community- based service providers was created to develop partnerships with health care providers as a means to solve for this common disconnect.
This coalition, the Twin Cities Aging Network, selected Turnlane to serve as the lead partner in developing Network capacity and strategic partnerships. Turnlane provided leadership through strategy development, building infrastructure and cross-Network processes, negotiation with health care partners, and assistance in the creation of a software tool for Network-wide use.
